Duplicate, repeated or mismatched submissions will be rejected, and you may not be able to edit or re-submit your information afterwards.
Notice for Certain Applicants
Applicant who have obtained their degree from local or foreign universities / institutions not belonging the universities indicated in the University Grant Commission official web site as mentioned under the section 1.1 v (b) of the G3 form.
Instructions & Guidelines for Candidates
- Fill in all required fields marked with * before submitting.
- Enter your Full Name exactly as it appears on your NIC and degree certificate.
- Enter your NIC Number without spaces or dashes (e.g. 199012345678).
- Enter your Exam Index Number as shown on your examination result sheet.
- Select your Exam Medium (Sinhala, Tamil, or English).
- Enter all your eligible subject details (up to 4) with each Subject Number. The Subject Names are auto-filled and locked.
- Select your Interview Date, Time Slot, and Panel No (1-6).
- Provide the Name of Degree Awarding Institute / University and Country of the Institute / University.
- Select the Medium of Degree (Sinhala, English, Tamil, or Other).
- Upload a clear, readable copy of your Transcript (PDF, JPG, or PNG, max 5MB).
- Upload a clear, readable copy of your Degree Certificate (PDF, JPG, or PNG, max 5MB).
- Do not upload any documents other than your bachelor's degree or its transcript documents.
- You may submit this form only once for your NIC / Exam Index Number (candidate number). Double-check everything before submitting.
- Enter a valid Email Address. A copy of your submitted details and your Reference Code will be emailed to you.
- You must check the Declaration box to confirm the information is true.
Important: Providing false, incorrect, or misleading information will result in immediate disqualification from the interview process.
